We have an exciting opportunity for a Band 7 Advanced Clinical Practitioner (ACP) committed to delivering high quality care to join our already dedicated and committed Care of older people Team. The ACP will be based on one of the Care of the elderly (COTE) wards at DRI or MMH. There will be opportunity to rotate into Frailty assessment unit (FAU) and other areas within the Division of Medicine.  The aim will be for some participation onto the hospital @ rota.

Consideration will be made to anyone who wishes to commence the Trainee Advanced Clinical Practitioner (TACP) role.  The TACP post will involve training for 2 years and will be paid at mid-point band 6, on successful completion of the academic modules and clinical competence ACP’s will be paid at a band 7.

COTE is part of the Division of Medicine at Doncaster & Bassetlaw Teaching Hospitals and provides high level care to patients referred from FAU. COTE wards provide high quality, timely and safe care through a bespoke pathway, helping to reduce length of stay, reduce admissions and improve patient experience.

As an experienced ACP, you will provide high quality clinical care for patients presenting with a variety of undifferentiated conditions, providing a comprehensive advanced clinical assessment, comprehensive geriatric assessment, requesting and interpreting investigations, decision making, provision of treatment, prescribing, evaluation of care and referral to other healthcare professionals if required.  Whilst working autonomously the ACP will always work within a multi professional team led by a Consultant.

We expect that you will be fully dedicated to person-centred care and that you will work collaboratively with our multi-disciplinary teams to ensure that patients receive a tailored management plan which addresses both current issues and proactively looks to prevent potential issues which may arise.

To be part of our team the successful candidate will be an autonomous practitioner ensuring that your practice is underpinned by the four pillars of advanced clinical practice, have recent experience of caring for patients in an acute setting, have excellent leadership and clinical skills, as well as evidence of continuing professional development, have drive, ambition, passion, a flexible approach and dedicated to working with frail elderly patients.

On appointment the successful candidates will be allocated a clinical and educational supervisor, access to eportfolio and will be supported to complete any educational or clinical training for their on-going CPD.
