An exciting opportunity has arisen to join the Trust as a Patient Safety Lead. We are looking for a highly motivated and committed Registered Healthcare Professional to join the Patient Safety Team at Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust.

The Trust is on a journey to be the “Safest Trust in England, outstanding in all we do”. The post holder has an opportunity to respond to patient safety incidents in an effective and proactive manner, ensuring the principles of just culture are embedded.

The post holder has a key role in the delivery of the National Patient Safety Strategy. You will be part of the team that leads investigations across the Trust. The successful applicant will show outstanding communication skills and demonstrate the ability to work collaboratively with a wide range of staff and specialties. Working as part of a team, you will work with the clinical teams to identify quality improvements through investigation of incidents and identification of emerging risks through incident reporting.

The successful candidate will be responsible for:

  • Leading Serious Incident Investigations as per the Serious Incident Policy in collaboration with the Division. Includes interviewing staff, visiting ward/departments and writing reports.
  • Proactive management of the Inquest processes working to support the work of the Coroner and represent the Trust’s interest at Coroners Court.
  • Scoping incidents when they happen and updating Datix accordingly.
  • Monitoring for the Division and attending relevant governance meetings to discuss Duty of Candour, SI actions, Risk registers and Open incidents.
  • Attending Serious Incident Panel, Inquest Panel and Patient Safety Review Group when required.
  • Provide the necessary input to the reports required for Trust Committees attending as necessary

The post holder will report to the Head of Patient Safety and Quality, and will have direct links to Deputy Director of Nursing – Patient Safety and Deputy Medical Director (Clinical Governance).
