We are now accepting self-referrals for the majority of conditions.

How do I refer?

You can self-refer into the service using the online referral form below. Please use this form if you are able as it will give us more information to make sure you are accessing the most appropriate service for your condition.

Alternatively, if you are unable to use the online referral form, contact our booking team on 01302 648205.

Please note, this phone won’t be manned and you will be asked to leave your name and contact telephone number. A member of the booking team will call you back to arrange an appointment.

Where will I be seen?

The majority of self-referrals will first have a telephone triage appointment.

What happens during the telephone triage appointment?

One of our musculoskeletal Physiotherapists who will discuss your symptoms and offer advice to help manage these.

Should you require a further appointment this will usually be arranged face to face at one of our Physiotherapy centres/sites (Locations).

Please be aware that there may be a wait following you initial triage appointment.

If we are unable to contact you at the agreed appointment time, you will be discharged and a further referral will be required.

How do I change my appointment?

If you need to re-arrange it or cancel your appointment, please let us know as soon as possible by contacting 01302 644200.

This will allow us to offer the appointment to someone else.

What happens if I need to be referred to another service?

If you require referral to an alternative service, our clinicians will be able to advise you appropriately.

How long will I be waiting for an appointment?

We hope to be able to offer you an initial self-referral telephone triage appointment within 5 working days of receiving your referral but this may vary during periods of high demand.

I can’t access the online referral form, what shall I do?

If you cannot complete the online form, please contact us on 01302 648205. where you can self-refer by leaving us a message.
