It is currently a very exciting time for Doncaster and Bassetlaw Teaching Hospitals (DBTH). Our Trust  places patient safety and patient experience as high priorities, contributing to the improvement in the quality of care provided to our patients. Maintaining the momentum of improvement over the last number of years, we aim to maximise learning opportunities that we find.

We seek applications from registered health professionals for this post to lead the patient safety investigation, learning and improvement of the quality of care and services we provide to the patients attending our hospitals. The successful candidate will be enthusiastic, well informed and knowledgeable in patient safety, investigation skills and contribute to the continuing improvement of quality in the Trust.

The successful candidate will be responsible for:

  • Leading Serious Incident Investigations as per the Serious Incident Policy in collaboration with an allocated Care Group Clinical Lead.
  • Proactive management of the inquest processes working to support the work of the Coroner and represent the Trust’s interests.
  • Monitoring of the Duty of Candour regulation requirements.
  • Advice on initial investigation and scoping actions for incidents, inquests and claims.
  • Provide line management to the Patient Safety Co-ordinator within the team.
  • Liaising with the Clinical Governance Lead, Head of Patient Safety & Experience and senior personnel within the Care Group to co-ordinate an ongoing patient safety quality improvement programme
  • Provide the necessary input to the reports required for Trust Committees attending as necessary
  • Liaison with CCG’s Quality Team and other provider organisations as necessary.

The post holder will report to the Head of Patient Safety & Experience, and will have direct links to the Deputy Director of Quality and Governance and Deputy Medical Director (Clinical Governance). Please note on completion of the organisational restructure the line management of this post will move into the Divisional structure.
