An exciting opportunity has arisen to join the Bassetlaw Diabetes Specialist Nurse Team.
The Diabetes Nurse Practitioner will be a key member of the Diabetes Multi-Disciplinary Team (MDT); working to implement the recommendations from nationally accepted guidelines, for example National Institute of Clinical Excellence (NICE).
The post holder will be responsible for the assessment of patients admitted to the Acute Care setting with a diabetes related episode and patients admitted for elective or emergency surgery with complex needs following MDT referral. Contributing to reducing length of stay and working towards reducing re-admissions to secondary care.
