Wet AMD Referral Form

We have introduced a new E-Referral form for the wet AMD service at DBTH. This is to enable fast track referrals to the service electronically.  The completed form must be sent via your  NHSmail account to the designated mailbox only: dbth.wetamd@nhs.net

Please note that NHS faxes were shut down on 31st March 2020.
