Young Adults Diabetes

The Young Adult Diabetes Team (Transitional Team) treat and manage patients with Type 1 and Type 2 Diabetes across Doncaster and Bassetlaw. Patients diagnosed with diabetes from the age of 16 will be referred to the Young Adult Diabetes Team and will remain with them until 24 years of age. The patient will be allocated a key diabetes nurse and consultant, with the additional support from a dietitian. We work closely with IAPT (Improving Access to Psychological Therapies) having regular fortnightly clinics and have strong links to other agencies.

We also assist with the Young Person consultant clinic’s within paediatrics, diabetic paediatric patients remain within this service until 18-19years of age, then get transferred to the Young Adult service. By transfer the patients will already know the Young Adult Diabetes team easing the transition and will have been allocated a new Young Adult nurse and consultant.

 

The Transitional team was developed in 2018 to help ease the transition from paediatric care to adult services.
