Referrals for our inpatient service are sent from the ward/department staff via our internal electronic documentation /reporting system. Wards/departments will be notified if a referral is incomplete to enable the full information to be provided. This will delay the process of the request for service.

All completed referrals will be triaged by the Skin Integrity team with 72 hours of receiving the referral. The referred case will be assigned to one of the following:

  • No skin integrity input required – recommended to refer to a different specialist/service/team e.g. Dermatology
  • No skin integrity input required – Ward/department to follow a Trust Pathway e.g. Doncaster Wide Skin Care Pathway for Moisture Related Skin Damage
  • Skin Integrity involvement required – Suitable for a virtual review and recommendations
  • Skin Integrity involvement required – Requires a face to face review non urgently (within 72 hours)
  • Skin Integrity involvement required – Requires a face to face review urgently (within 24 hours)
