Referrals for our outpatient service are accepted from a healthcare professional via our electronic referral form sent via email (link for from and pathway). The sender will be notified if a referral is incomplete to enable the full information to be provided prior to the referral being accepted. This will delay the process of the request for service.

All completed referrals will be triaged by the Skin Integrity team with 72 hours of receiving the referral. The referred case will be assigned to one of the following:

  • No skin integrity input required due to requirements needed – recommended to refer to a different specialist/service/team e.g. Dermatology
  • No skin integrity input required due to exclusion criteria – people that are not currently having active input from a consultant regarding the wound.
  • Skin Integrity involvement required – review non urgently (within 7 – 10 days)
  • Skin Integrity involvement required – review non urgently (within 1 – 6 days)
