Privacy Notice for Children

We want to make sure that everyone understands how and why we need their information and that they know what their rights are. This privacy notice tells you what happens to the information we collect about you whenever you come to see us. It also tells you how we make sure your information is kept safe.

What information do we collect?

We may ask for or hold information about you which will be used to help us to look after you when you are in hospital or coming to see us for an appointment. We might keep:

  • Basic details such as name, address, date of birth
  • Information about the person who will usually come with you to your appointments and who help us make decisions about your care.
  • Personal information such as your religion, allergies or health conditions
  • Details about any appointments you’ve had with us or your Doctor (GP)
  • Details and records about your treatment and care and why you have come to see us.
  • The results of any tests you’ve had.
  • Information from people who care for you and know you well, such as health professionals (Doctors and Nurses) and relatives.

What do we use your information for?

We need to collect some information about you to make sure we can give you the best care possible. It is important for us to have a complete picture, as this information helps everyone who you might meet in hospital to look after you. Each time you come to see us we will write down things you tell us, things we tell you and any medicines we may need to give you.

Who might we share your information with?

Other organisations involved in your care and safety, for example:

  • Other hospitals
  • Your GP (Doctor)
  • Ambulance Services
  • Social Services
  • Education Services (Schools)
  • The Police
  • Commissioners who pay us to provide a service
  • Hospital inspectors, who are known as the Care Quality Commission. They carry out regular inspections to make sure that we are treating and caring for you in the best way we can. The people who inspect us may ask to see a small number of health records for our patients. They check that notes are written clearly and are kept safe to ensure that we are recording and storing your information correctly.

Where we can, we will try to ask you if it is okay to do this but sometimes we might need to share this information without asking you first. This may be because we are not able to ask you or because it needs to be done quickly to help you.

What are your Rights?

  • You can tell us when you do not want your information shared. This could be with your parents, carers or others, and we will always talk this through with you before deciding.
  • If you tell us not to share your information, we will make sure we don’t wherever we can.
  • We will only share your information if the law tells us that we have to.
  • You can ask to see the records that relate to you and only you and you can request copies. You should ask for your information in writing and include your full name, address, birthday and the number known as the NHS number. You can speak to any of the staff looking after you about this.
